WELCOME TO TIVERTON MIDDLE SCHOOL ATHLETICS

 ATHLETIC PERMISSION SLIP

All of our sports require athletes to have the mandated paperwork submitted to the coordinating head coach and received by Mr. Grant (Athletic Director) prior to participating.   For your convenience, type directly on the form and submit electronically via email to your athlete's coach (please include your child's name within the subject).   The required state physical form can be found here

*Only one athletic packet is required per school year, per student. If your student participates in more than one sport, you will not need a second packet notarized. Free notary services are available in any office at the Tiverton Town Hall during their business hours of 8:30 A.M. to 4:00 P.M. Monday through Friday. Tiverton Town Hall is located at 43 Highland Road. Parking is available.

TIVERTON MIDDLE SCHOOL ATHLETIC BOOSTERS

Volunteers are needed to assist with our home events by directing athletes, fundraising efforts, running a snack stand and in many other ways. Please consider giving just one hour of your time this season. Teamwork makes the dream work. All volunteers are required to obtain a Background Check Investigation (BCI) to assist with our program. For volunteer opportunities within our athletics, please contact the main office at 401-624-6668.  TMS Athletics provide our students a social, emotional and recreational outlet that is imperative to their development. Thank you for doing your part as a parent/community to set up our students for a successful and healthy lifestyle.
