This form should be used to petition the Senior Project Advisory Board for:

• Change of your Senior Project topic
• Early start for your Senior Project (summer prior to senior year)
• Deadline extensions (must be submitted prior to deadline)
• Appeal of a Senior Project topic denial

Form should be completed and returned to your English Instructor as soon as possible.

The Senior Project Advisory Board will notify student of action.
