Fall Season News:  Required Forms Prior to Participation

 All athletes must have the following forms filled out prior to the start of practice or they are not able to participate until they are completed
 
              A   Must have a current physical
              B   Drug Alcohol Policy
              C   RI Interscholastic league wavier form signed and
                    Notarized
              D   Insurance form  
              E   Emergency medical release form
 
 
All of these forms are available in the main office
